CG 12-10498 Health Equity Summit

The California Department of Public Health (CDPH), California Tobacco Control   Program (CTCP) requests proposals from qualified individuals or organizations that can provide full services to plan, develop, and facilitate a Health Equity Summit for the purpose of developing a statewide written strategy to decrease tobacco-related disparities in California.  The Health Equity Summit will be developed in partnership with an external workgroup convened by CTCP.  Following the Summit, the contractor will be required to compile a strategy report based on the input received from the Health Equity Summit.

• An optional Informational Online WebEx™ & Teleconference will be held on December 13, 2012 from 10:00 a.m. until 11:00 a.m. PT. Prospective proposers intending to submit a proposal are strongly encouraged to participate. The purpose of the Informational online meeting is to provide potential proposers with an opportunity to ask questions about the preparation and submission of the proposal. This Online WebEx™ & Teleconference is the only medium available for questions about this RFP .

 Only proposals from state and local government or nonprofit organizations will be considered.

The anticipated funding level for the seven-month period tentatively beginning February 1, 2013 and ending August 31, 2013 is not to exceed $50,000 to complete services described in this Request for Proposals.  Although not anticipated, CTCP may increase funding, not to exceed $10,000, during the agreement term if changes occur in state or federal funding, Legislation, court action, or other administrative actions which impact the Health Equity Summit.  Funding for this Request for Proposal is subject to continued appropriation of revenues by the legislature to CDPH/CTCP and by the Centers for Disease Control and Prevention.  If funding is not available or if it is reduced, CDPH/CTCP may terminate the agreement or amend the agreement to reflect reduced funding and deliverables.
